At your first visit to our office, you will be asked to provide us with some basic medical history, family information, and insurance information. Please plan to arrive 15 minutes early to allow time to complete this information. What do we need to bring to our appointment?  Please bring parent identification and insurance cards to all of your appointments.  If you have legal custody or guardianship of the patient, please bring the appropriate documentation.  This information will also be required by the hospital if surgery will be necessary.  Also, please bring any relevant medical information you may have (test results, X-ray and pathology results) that may be helpful information in fully evaluating your child's problem.

How do I make an appointment?  Please call (702)233-8101 to schedule an appointment with either doctor.

Do I need a referral to come to your offices?  Your insurance plan may require you to obtain a referral from your child's primary care doctor before allowing you to see us.  We may be able to assist you with this.

What payment is expected?  All insurance policies differ, and it is your responsibility to review your own individual plan.  Many insurance plans require a co-payment for an office visit or surgical procedure and/or require you to meet a deductible amount before insurance will pay for services.  Co-payments and unmet deductibles are due at the time the service is provided.  We are happy to help and welcome calls if there are any questions.
