What do we need to bring to our appointment?  Please bring parent identification and insurance cards to all of your appointments.  If you have legal custody or guardianship of the patient, please bring the appropriate documentation.  This information will also be required by the hospital if surgery will be necessary.  Also, please bring any relevant medical information you may have (test results, X-ray and pathology results) that may be helpful information in fully evaluating your child's problem.

How do I make an appointment?  Please call 233-8101to schedule an appointment with either doctor.

Do I need a referral to come to your offices?  Your insurance plan may require you to obtain a referral from your child's primary care doctor before allowing you to see us.  We may be able to assist you with this.

What payment is expected?  All insurance policies differ, and it is your responsibility to review your own individual plan.  Many insurance plans require a co-payment for an office visit or surgical procedure and/or require you to meet a deductible amount before insurance will pay for services.  Co-payments and un-met deductibles are due at the time the service is provided.  As a courtesy to you, we will bill your insurance company for any balances.  Once the insurance has paid their portion of a charge, the family will receive a statement for their portion (if applicable).  Billing questions are gladly handled right in our office.  Call us during business hours at 233-8101 and ask for the billing manager.
